The unit test on adding and subtracting fractions with unlike denominators. Pages 1 and 2 are Form A. Pages 3 and 4 are Form B: the same questions in the same order with different numbers, for a retake, a make-up or a second seating. Each form has 11 questions, one point each, and a Score line on its first page. Every fraction added or subtracted has a denominator of 2, 3, 4, 5, 6, 8, 10 or 12, and no common denominator is over 60. Questions 1 to 10 cover the Common Core lessons; question 11 (prime numbers and a factor tree, in two parts) is for the states that teach it in 5th grade, so a class that skips that lesson can stop at question 10 and score out of 10. The pre-check and the unit review are packs of their own.
Give Form A the day after the unit review. Question 2 renames both fractions with the least common denominator before comparing; the two fractions are close, so a guess by benchmarks will not do. Questions 4 to 7 ask for answers in simplest form: on Form A a student who stops at 5/10 in question 4 or 3 3/12 in question 6 has the adding right and the last step to practice. Where an answer is a mixed number, a fraction greater than 1 in lowest terms is right too, as the keys show. Question 8 shows two answers to judge with 0, one half and 1: the right one is near the estimate, the wrong one is far from it. No question asks a problem from the unit's exit tickets or the practice sheets its lessons use. The answer keys name each question's skill, so a missed question points to the lesson to reteach.
Use Form B as a check after the unit. Ask your child to add 1/2 + 1/3 out loud: why sixths, how many sixths each fraction is, and the sum.

The unit test on adding and subtracting fractions and mixed numbers with unlike denominators, in two forms. Students simplify with the greatest common factor, rename and compare with the least common denominator, add and subtract fractions and mixed numbers, estimate with benchmarks and solve story problems.
