The unit test for the fourteenth unit of 5th grade, the coordinate plane and patterns. Pages 1 and 2 are Form A. Pages 3 and 4 are Form B: the same questions in the same order with different numbers, for a retake, a make-up or a second seating. Each form has 10 questions, one point each, and a Score line on its first page. Questions 1 to 6 are the Common Core part of the unit; questions 7 to 10 (input-output rules, adding or multiplying, patterns with decimals and fractions) are for the states that teach them in this unit.
Give Form A the day after the unit review. Have sharp pencils ready for plotting. Most questions hold more than one part, such as three points to read or plot, or a table to fill in and a rule to write: give the point when all are right, or score them by the part if you like. Where a rule can be written more than one way, the key shows one way and says so. Leave out questions 7 to 10 if your class does not learn them and score out of 6. No question asks a problem from the unit's exit tickets or the practice sheets. The answer keys name each question's skill, so a missed question points to the lesson to reteach.
Use Form B as a check after the unit. Ask your child to say how to plot a point aloud, across first and then up, and to say the rule of a table before writing it.

The unit test on the coordinate plane and number patterns in two forms, from reading ordered pairs to graphing the pairs of two patterns. Students read and plot points, find lengths along grid lines, read a map, make two patterns from two rules and graph their pairs.
